1. Participation Agreement and Acknowledgment of Risk

Hebron Sports Development Initiative (HSDI) provides youth athletic opportunities intended to promote physical development, teamwork, discipline, sportsmanship, and community participation.

By registering a child in HSDI Basketball, the parent or legal guardian acknowledges the following:

Program participation

HSDI Basketball includes organized practices, skill development, conditioning appropriate to the athlete’s age, games, team activities, and related events.

The program operates through the following divisions:

  • Kindergarten–2nd Grade: PSA Recreational Basketball

  • 3rd–6th Grade: PSA Select Basketball

Team assignments and competition participation are determined by HSDI in coordination with applicable league requirements.

Inherent risks

Basketball involves physical activity and contact with other participants, equipment, and playing surfaces.

Potential risks include falls, collisions, sprains, fractures, concussions, dehydration, illness, and other injuries. Although HSDI seeks to provide an appropriately supervised environment, not all risks can be eliminated.

Parent acknowledgment

I acknowledge that:

  1. My child is voluntarily participating in HSDI Basketball with my permission.

  2. I understand that basketball involves inherent physical risks, including the possibility of serious injury.

  3. I will inform HSDI of relevant medical conditions, injuries, allergies, or restrictions that could affect safe participation.

  4. I understand that HSDI may temporarily restrict participation when reasonably necessary for safety.

  5. I understand that participation does not guarantee playing time, a particular position, team assignment, competitive results, or selection for a specific roster.

I have reviewed the nature of the program and authorize my child to participate subject to the applicable policies and laws.

Required acknowledgment:

☐ I have read and understand the HSDI Participation Agreement and Acknowledgment of Risk, and I authorize my child to participate.

I certify that I am the parent or legal guardian of the athlete identified in the registration and have the legal authority to authorize participation.

I grant permission for the athlete to participate in HSDI Basketball practices, games, training sessions, and related supervised program activities.

Parent responsibilities

I agree to provide accurate registration and emergency contact information, communicate relevant health or safety concerns, and comply with reasonable HSDI participation and facility requirements.

I understand that HSDI is not responsible for providing personal medical insurance unless expressly stated in a separate written agreement.

Liability provisions

To the extent permitted by applicable Texas law, I agree that I will not hold HSDI responsible solely because an injury results from the ordinary inherent risks of voluntary basketball participation.

To the extent legally enforceable, I release HSDI and its directors, officers, coaches, volunteers, and authorized representatives from claims belonging to me personally arising from ordinary negligence in connection with my child’s participation.

This provision does not purport to waive my child’s independent legal rights or claims, waive rights that cannot lawfully be waived, or release liability for gross negligence, reckless conduct, or intentional misconduct.

I understand that the legal effect of this agreement is governed by applicable law.

Required acknowledgment:

☐ I confirm that I am authorized to register this athlete, consent to participation, and have read the Parent/Guardian Consent and Liability Release.

3. Emergency Medical Authorization

The safety and welfare of participating athletes are priorities of HSDI.

Parents and legal guardians are responsible for supplying current emergency contact information and notifying HSDI of relevant health conditions or activity restrictions.

Emergency procedures

If an athlete experiences an injury, illness, or other medical emergency during an HSDI activity, authorized personnel may:

  1. Provide reasonable basic first aid within their training and capabilities.

  2. Contact the parent, guardian, or designated emergency contact.

  3. Contact emergency medical services when warranted.

  4. Communicate relevant emergency information to responding medical professionals.

  5. Arrange for emergency transportation when medically necessary.

Authorization

If I cannot be reached promptly, I authorize HSDI representatives to summon emergency medical assistance and provide necessary information to licensed medical professionals.

I authorize emergency medical professionals to evaluate and provide treatment to my child when permitted by applicable law and medically necessary.

I understand that this authorization does not independently authorize HSDI coaches or volunteers to administer prescription medication or perform medical procedures beyond their training or lawful authority.

I understand that I am responsible for medical expenses associated with my child’s treatment, subject to any applicable insurance coverage or other legal responsibility.

Medical information

I agree to disclose information reasonably necessary for safe participation, including relevant allergies, medical conditions, activity restrictions, or emergency care instructions.

Such information will be handled as confidential registration information and shared only as reasonably necessary for safety, emergency response, or program administration.

Required acknowledgment:

☐ I authorize HSDI to obtain emergency assistance for my child as described above and confirm that the emergency information I provided is accurate.

4. Athlete and Parent Code of Conduct

HSDI Basketball is intended to develop young athletes through disciplined instruction, teamwork, accountability, and sportsmanship.

Participation requires respectful conduct by athletes, parents, guardians, and spectators.

Athlete expectations

Athletes are expected to:

  1. Treat coaches, teammates, opponents, officials, and spectators respectfully.

  2. Follow reasonable coaching instructions and safety requirements.

  3. Attend scheduled practices and games when reasonably possible and communicate absences.

  4. Demonstrate effort, teamwork, and appropriate sportsmanship.

  5. Refrain from bullying, harassment, threats, fighting, or discriminatory behavior.

  6. Respect facilities, equipment, and other participants’ property.

Parent and guardian expectations

Parents and guardians agree to:

  1. Support the developmental purpose of HSDI Basketball.

  2. Treat coaches, officials, athletes, and other families respectfully.

  3. Avoid abusive language, intimidation, harassment, or confrontations.

  4. Refrain from interfering with coaching decisions or officiating.

  5. Communicate concerns through appropriate HSDI channels.

  6. Ensure athletes arrive with appropriate equipment and are picked up promptly.

  7. Follow facility rules and reasonable event procedures.

Coaching and team decisions

Coaching staff may make decisions regarding practice structure, positions, substitutions, playing time, and team responsibilities consistent with league rules and HSDI policies.

Parents may raise concerns respectfully, but registration does not guarantee a specific coach, roster, role, or amount of playing time.

Discipline

Violations may result in verbal warnings, parent conferences, temporary restrictions, suspension, or removal from the program.

HSDI may take immediate action when conduct creates a significant safety risk.

When practical, families will be informed of disciplinary decisions and given an opportunity to discuss concerns with HSDI leadership.

Required acknowledgment:

☐ I have reviewed the HSDI Athlete and Parent Code of Conduct and agree to follow these expectations.

5. Basketball Registration, Payment, and Refund Policy

Registration fees

The HSDI Basketball registration fee is $225 per athlete.

The registration fee supports program administration, league participation, team operations, and other applicable program expenses.

Uniforms are separate from the registration fee unless HSDI expressly states otherwise in writing.

Registration is subject to eligibility requirements, available roster space, and applicable league rules.

Payment processing

Online payments are processed through Stripe Checkout.

Payment information is entered through Stripe’s secure payment interface. HSDI does not require parents to provide complete card numbers directly through its registration form.

Submitting registration information without completing payment does not establish a paid registration.

Refund eligibility

Before team placement

A parent or legal guardian may request a full refund of the HSDI Basketball registration fee before the athlete has been assigned to a team.

For purposes of this policy, team placement occurs when HSDI formally assigns the athlete to a specific team roster and records that assignment.

A registration application, payment confirmation, or initial eligibility review does not, by itself, constitute team placement.

After team placement

Once an athlete has been assigned to a team, the registration fee becomes nonrefundable, except as required by applicable law or as expressly provided below.

The following generally do not qualify for refunds after team placement:

  • Voluntary withdrawal.

  • Changes in family schedules or availability.

  • Missed practices or games.

  • Dissatisfaction with team assignment, playing time, or coaching decisions.

  • Athlete suspension or removal for documented conduct violations.

Program cancellation

If HSDI cancels an athlete’s Basketball program or cannot offer the athlete a team placement, HSDI will provide a full refund of the HSDI registration fee.

If a season is materially interrupted after it begins, HSDI will determine any appropriate refund or credit based on the circumstances, services already provided, applicable contractual obligations, and governing law.

Nothing in this policy limits refunds or other remedies required by law.

Refund requests

Refund requests must be submitted in writing to:

info@hebronhsdi.org

Requests should include the athlete’s name, parent or guardian name, and registration reference.

HSDI will review the request and communicate its determination. Approved card refunds will ordinarily be issued through the original payment method, subject to payment processor processing times.

Payment disputes

Families are encouraged to contact HSDI regarding payment errors or registration concerns before initiating a dispute. This does not restrict any legal right to dispute a charge with a payment provider.

Required acknowledgment:

☐ I understand that the Basketball registration fee is $225 per athlete, that full refunds are available before team placement, and that fees become nonrefundable after team placement except as stated in this policy or required by law.

6. Privacy Policy and Registration Information Disclosure

Information collected

HSDI collects information reasonably necessary to register athletes, administer programs, communicate with families, and support participant safety.

This may include:

  • Athletic experience and sport-specific registration details.

  • Emergency contacts and relevant health or accommodation information.

  • Registration agreements and acknowledgment records.

  • Payment transaction references and payment status.

  3. Communicate program schedules, changes, and administrative notices.

  4. Respond to emergencies and address reasonable safety needs.

  5. Process payments, refunds, and financial records.

  6. Maintain appropriate records and comply with legal obligations.

  7. Protect registration systems against unauthorized access and abuse.

Sharing of information

HSDI may share limited information with authorized coaches, program administrators, league operators, facility partners, payment processors, and technology providers when reasonably necessary to operate the program.

Only information reasonably necessary for each purpose should be shared.

HSDI does not authorize the sale of children’s personal information.

Information security

HSDI uses third-party technology services, including Framer, Supabase, and Stripe, in connection with its website, registration records, and payments.

HSDI will use reasonable administrative, technical, and organizational safeguards intended to protect registration information from unauthorized access, disclosure, alteration, or loss.

Access to personal information will be limited to authorized individuals who require it for legitimate program responsibilities.

No electronic system can guarantee absolute security.

Retention and deletion

HSDI retains registration and payment records for as long as reasonably necessary for program administration, legal compliance, financial accounting, dispute resolution, and legitimate organizational purposes.

Information no longer required will be deleted or securely de-identified in accordance with applicable retention obligations.

Parent access and corrections

Parents and legal guardians may request to review, correct, or seek deletion of their child’s registration information by contacting HSDI.

Requests will be evaluated in accordance with applicable law and legitimate record-retention requirements.

Children’s privacy

The HSDI registration process is intended to be completed by parents or legal guardians, not independently by children.

Children should not submit personal registration information without the involvement and authorization of their parent or legal guardian.

HSDI will not intentionally use children’s registration information for unrelated behavioral advertising.

  • Community outreach and organizational publications.

  • Historical and archival documentation.

HSDI will not intentionally publish an athlete’s private contact information, medical information, or home address with media content.

Optional parent authorization

I understand that media participation is voluntary and is not a condition of Basketball registration.

If I grant permission, I authorize HSDI to use photographs, video, and audio recordings featuring my child for the organizational purposes described above without compensation.

I understand that previously published material may not always be fully retractable from third-party platforms or materials already distributed.

I may withdraw permission for future HSDI-controlled use by submitting a written request to info@hebronhsdi.org.

If I decline, HSDI will make reasonable efforts to exclude my child from identifiable promotional photographs and videos under its control.

Optional selection — one response required:

☐ YES — I authorize HSDI to use photographs and recordings of my child as described.

☐ NO — I do not authorize identifiable promotional use of photographs or recordings of my child.

Declining media authorization will not affect registration eligibility or team participation.
